How to get a Provisional Licence

Provisional License

You can apply for additional provisional entitlements to be added to your licence when you have passed your category B (car driving licence)

For all LGV and PCV provisional licences you must apply to DVLA using the following forms:
  • D2 - Application for a driving licence. This form is used to apply for the required provisional licence.
  • D4 - Medical Report. Your doctor must complete this form and an eyesight examination at an optician may also be necessary.
These forms can be requested from DVLA and when they have been completed they must be sent together with your existing driving licence to:

For LGV provisional licence select category C, this will give you:

Medium Size Vehicles
C1     Vehicles between3500kg and 7500kg with a trailer up to 750kg MAM*
C1+E Vehicles between 3500kg and 7500kg with a trailer over 750kg (combined MAM* cannot exceed 12000kg)

Large Vehicles
C      Vehicles over 3500kg with a trailer up to 750kg MAM*
C+E  Vehicles over 3500kg with a trailer over 750kg MAM*

For PCV provisional licence select category D, this will give you:

Minibuses
D1     Vehicles with no more than 16 passenger seats with a maximum length of 8 metres and a trailer up to 750kg MAM
D1+E Vehicles in category D1 with a trailer over 750kg MAM* (combined MAM cannot exceed 12000kg) 

Buses
D       Vehicles with more than 8 passenger seats with a trailer up to 750kg MAM* 
D+E  Vehicles in category D with a trailer over 750kg MAM* 

MAM* Maximum Authorised Mass, the combined weight of the vehicle and load 

When you receive your provisional licence you may commence supervised driving tuition.
